Superior HealthPlan has rolled out a new tool aimed at tackling the barriers that keep Texans from accessing vital services like healthcare, food, and housing. This fresh approach comes with a screening tool that connects individuals to local community resources, helping them get the help they need beyond the doctor’s office.
The goal is simple: Superior HealthPlan, a top managed care organization in Texas, wants to make it easier for people to navigate non-medical challenges that often stand in the way of good health—whether that’s finding reliable transportation to appointments, securing affordable housing, or putting food on the table.
“This screening will be conducted by our clinical team,” said Dr. Susan Mills, Senior Vice President of Population Health and Clinical Outcomes at Superior HealthPlan. “We want to identify what people need right away and connect them with local resources as quickly as possible. Afterward, we’ll make sure they got the services and then build their ongoing care around those needs.”
The new assessment, which launched on September 1st, is part of a broader health risk evaluation that members with complex care needs are already receiving. Once the screening is done, Superior’s staff steps in to help members find community support through a helpful online portal, superiorhealthplan.findhelp.com, available to all Texans. This tool links people to services such as food pantries, job training programs, and even medical care at reduced or no cost.
Since January 2024, more than 800 members have already used this platform to get the help they need, and those numbers are expected to grow fast. With this new assessment tool in play, Superior HealthPlan is gearing up to see a big jump in the number of people who will benefit.
“This is local healthcare at its best,” said Dr. Mills. “Our employees live in the same communities as our members, which gives them the inside track on finding the right resources to break down those barriers to care. We’re excited about this new tool because it’s going to make a huge difference for so many Texans.”
