In a groundbreaking initiative, ‘Ohana Health Plan has teamed up with Hope Services Hawaii to launch a revolutionary residential program in Hilo. This innovative project aims to provide transitional housing and comprehensive healthcare support for some of Hawaii’s most vulnerable residents—those who are homeless and dealing with complex medical and behavioral health needs.
On January 14, 2025, the grand opening of this six-bed facility marked a significant step forward for community health. The home, exclusively for ‘Ohana Health Plan members, offers integrated care in a secure environment, helping individuals stabilize and work towards better health over an average one-year stay.
The program, a first-of-its-kind in Hawaii, bridges a critical gap in services. Traditional subsidized housing often falls short for those with severe health challenges, but this new approach aims to change that narrative. By offering concentrated access to both housing and healthcare, the program is expected to reduce emergency room visits and hospital stays, ultimately leading to better health outcomes and cost savings.

The property, owned by the County of Hawaii, is part of an existing lease agreement with Hope Services Hawaii, which provides emergency shelter for those experiencing homelessness. This new program extends those services by integrating health care into the housing solution.
The opening ceremony saw community leaders, including Hawai‘i Island Mayor Kimo Alameda, joining hands to celebrate this milestone. Their support underscores the community’s commitment to innovative solutions for homelessness and healthcare challenges.
